Chronic diseases such as diabetes, high blood pressure and high blood cholesterol remain one of Singapore’s leading public health concerns, with approximately 1 in 3 residents living with hyperlipidaemia or hypertension1. These conditions can develop without noticeable symptoms, which may delay diagnosis and treatment2.
Diabetes alone affects more than 400,000 people today, and is projected to affect one million people by 2050, with a rising prevalence in those under the age of 403. Early detection and timely management can help reduce the risk of complications and support better long-term health outcomes4.

Bringing Early Detection Closer to the Community
Not everyone undergoes regular health screening, and chronic conditions may also go insufficiently monitored or managed. Community-based screening can help bridge this gap by bringing health checks closer to where people live and work, providing opportunities to identify potential health risks and encourage earlier action.
However, screening is only the first step. Appropriate follow-up and clinical care are important to help individuals understand their results and manage identified health risks.
Through this strengthened partnership, SG Diagnostics and Raffles Medical are connecting community outreach with primary care. SG Diagnostics leverages its expertise in community outreach and rapid point-of-care diagnostics to support access to early health assessments, while Raffles Medical provides accessible primary care services for further clinical evaluation, diagnosis and, where appropriate, long-term chronic disease management.
By connecting early detection with appropriate primary care follow-up, the collaboration aims to encourage earlier action, reduce barriers to seeking care and support greater continuity in chronic disease prevention and management.
